DVR Ver 4 interface has extra step to turn on audio.
select 'Local' to use RCA audio input in the DVR. Default is 'Camera'
This setting is not on IE remote configuration.

And, select "Video& Audio" for Main Stream(Continuous), Main Stream(Event) and Sub Stream.
